How to Schedule an Appointment as a New Patient:

Call the clinic at (706) 922-0600 or simply come by our office.
To make your visit as quick as possible we have put our Intake form and medical history form on our website.  (The links are below.)  Please print these forms, fill them out and bring them with you to your appointment.

Paying for Your Visit:

While we are a non-profit health clinic, we are not a free clinic. 
Uninsured patients are welcome and will be put on a sliding fee scale starting at $25. 
The prices on the fee scale vary based on your income and the size of your household. 
If you apply for the sliding fee program, you will need to bring proof of income on your first visit.
We are also open to Medicaid and Medicare patients and accept most insurance plans.
