How to Schedule an Appointment as a New Patient:

Christ Community Health Services is not a walk-in clinic. All new patients are required to attend our New Patient Orientation. Call us at (706) 922-0600 to find out when the next orientation session is scheduled.

Our patient Intake form and medical history form can be downloaded from the bottom of this web page.  (The links are below.)  Please print these forms, fill them out and bring them with you to your appointment.


Paying for Your Visit:

While we are a non-profit health clinic, we are not a free clinic. 
Uninsured patients are charged on a sliding fee scale starting at $25 for an office visit.
Any lab work or other tests are not included in the $25 office visit charge. 
The prices on the fee scale vary based on your income and the size of your household. 
If you apply for the sliding fee program, you will need to bring proof of income on your first visit.
We also accept Medicaid, Medicare and most insurance plans.
